The deadline for reimbursement requests for annual-prepaid memberships passed on September 1, 2022.

Please submit your reimbursement request for the 2022/2023 academic year using the form below.

Your required paperwork by the deadline below (depending on how you pay for your membership):

  • Annually/Prepaid – Deadline: September 1, 2022 
  • Monthly – Deadline: June 1, 2023 (i.e. AFTER you’ve paid for 10-consecutive months within the current academic year)

The form is now closed and we will re-open the form in April 2023 to accept month-to-month reimbursement requests.

If your membership is “off-cycle” and you’ve renewed mid-year, please contact to determine if you qualify for the up to $100 reimbursement.
